IMPORTANT HEALTH COVERAGE TAX DOCUMENTS

1095-C Affordable Care Act Forms are available upon request

Under the Affordable Care Act, certain information is reported to the IRS regarding your medical insurance coverage (Forms 1095-B from the insurance provider and 1095-C from the employer). The IRS allows these forms to be sent “Upon Request” rather than mailed.  Requests for 1095-B and 1095-C tax forms must be fulfilled by January 31 or 30 days after the date of the request, whichever is later.

Forms 1095-B are provided by the insurance provider(s) to those enrolled during the year and are generally mailed directly to employees.

Forms 1095-C for the most recent calendar year(which provide information showing available coverage regardless of whether you choose to enroll or not) are available upon request.
